	passport valid for 6 months and with at least 1 empty page.
	three copies of filled out application form. (The form requires Abobe Acrobat.)
	three (3) passport size photos (2" x 2")
	valid reservation from a travel agency if travel as a tourist or letter from your employer stating the reason and exact dates of the trip if travel for business.
	a prepaid FedEx/Xpresspost airbill, if you want your visa couriered back to you
	cash, check or money order for the amount specified below, depending on visa service
