Please provide us with the following information to acquire an entry visa for Ghana:

	passport valid for 6 months and with at least 1 empty page and without any Israeli visas, seals or stamps.
	4 copies of a filled out application form. (The form requires Abobe Acrobat.)
	four (4) passport size photos (2" x 2")
	for tourist visa: detailed itinery print-out of your trip and a copyof the invitation from your host; for business visa: letter from your Canadian employer and invitation letter from host company in Ghana.
	flight confirmation from the Travel agency.
	an International Certificate of Vaccination for Yellow Fever.
	a prepaid FedEx/Xpresspost airbill, if you want your visa couriered back to you
	cash, check or money order for the amount specified below, depending on visa service
