	passport valid for 6 months and with at least 1 empty page.
	a filled out application form. (The form requires Abobe Acrobat.)
	one passport size photo (2" x 2")
	a prepaid FedEx/Xpresspost airbill, if you want your visa couriered back to you
	cash, check or money order for the amount specified below, depending on visa service
