Please provide us with the following information to acquire an entry visa for Venezuela:

	passport valid for 6 months and with at least 1 empty page.
	filled out application form. (The form requires Abobe Acrobat.)
	two (2) passport size photos (2" x 2")
	document that proves applicant's address in Canada (usually copy of Driver's Licence).
	letter from the applicant's bank, indicating the type, number of the account, average balance and date it was opened.
	letter from the applicant's employer, stating the position and date the employment started.
	a prepaid FedEx/Xpresspost airbill, if you want your visa couriered back to you
	cash, check or money order for the amount specified below, depending on visa service
